Please have your child’s pediatrician fax a script for OT evaluation and treatment, your child’s diagnosis code(s), and demographic information to 1-800-470-1905 for OT attention to Kaitlyn Cavins. You may also have your pediatrician use our individualized form, and indicate Made Resilient or First Available.

For private-pay families, the cost of an OT evaluation is $175, and the cost of a one-hour individual treatment session is $100. For families who wish to use their in-network insurance plan, cost ranges based on your specific insurance plan, co-payment, and deductible. Kaitlyn has been training in the Masgutova Neurosensorimotor Reflex Integration (MNRI) technique since 2023. MNRI is an integrative, developmental approach to restore nervous system balance and harmony through reflex integration. Clients are currently scheduled on a standing-appointment model. Due to the extent of our waiting list for standing appointment times, I offer cancellation appointments on a first-come, first-serve basis when current clients cancel an appointment. If you are interested in being on the cancellation list for random, intermittent appointments while waiting for a standing appointment time, please indicate that at the time of your evaluation or reach out to Kaitlyn directly.

Most standing appointment slots are one-hour appointments. Appointment slots either occur weekly or bi-weekly.

Evaluations are offered weekly on Wednesdays at 11AM.
